LIPOMATOUS CAUSES OF CORD TETHERING
A tethered cord can be caused by a number of abnormalities.
- Fatty Filum and Lipomas
- Spinal dysraphism
- Diastematomyelia
- Caudal regression and sacral agenesis.
- And a condition called a Tight Filum Syndrome or Occult Tethering.
This post looks at two abnormalities
- Fatty Filum
- Spinal Lipomas particularly of the Conus / Filum and distal thecal sac.
FATTY FILUM
|
Image Above: Thin linear fat ( blue arrows) in the filum terminale.
- Mostly these are asymptomatic
- But can be associated with tethering of the cord and a low conus
- Or a tight filum syndrome

SPINAL LIPOMAS
|
Spinal Lipomas are a common cause of tethering
- Can be Intra Dural, Extra Dural, a combination of the two or rarely intra medullary.
- May or may not result in cord tethering.
- Will follow fat signal on all sequences, fat saturate completely and dont enhance.
- May contain nerve roots traversing through them.
Image Above: Intra dural extra medullary lipoma ( blue arrows)surrounding the conus . The lipoma will have nerve roots traversing through it. No tethering as conus lies in normal position.

Image Above: Large extra dural lipoma ( blue arrows) but with intra medullary extension ( yellow arrows).
